NEW PATIENTS: 

1.  Please complete all forms & questionnaires available prior to your first appointment.  Click on the Questionnaire's button to the right. 

2.  Upload a profile photo of your face and the front/back of your insurance card.  

3.  Update your preferred pharmacy listing, medication list & supplement list.  Click on the buttons to the right.

Please feel free to contact the office with any questions or issues logging into the patient portal.  


We strive to respond to all patient portal messages within 48 hours during regular business hours. However, if you are experiencing a medical emergency, please call 911 or proceed to the nearest emergency department immediately.

If you are receiving an error message that there is no matching medication or supplement found, please send a secure message. to your provider with details about the medication or supplement you want to add.

If you are experiencing some other issue, please let us know what issue you are having regarding medications and supplements. The more detailed you are in your description the better we can help you.
